MEDICAL EVALUATION REQUEST FORM

Dear Patient, you may bring your personal health situation to our attention by using the data collection form and attaching any analysis and examination results you may already have. You can attach them to the form or send them later to: [email protected].

Our team will meet with you, completely free of charge, to evaluate your documentation, in order to understand whether our structure is in all respects able to provide you with concrete help and, if so, which professionals from our technical team, on the basis of their specific skills, should be in charge of following you on your path.

Once this has been defined, our Information and Consultancy Manager will contact you to explain what our first meeting might consist of, and to let you know all the details, including the duration of the visits and the costs.

At this point, if you decide you would like to schedule an initial visit to our facility, you can consult our schedule availability with the Reservations Manager.
